Service and Referral
At no cost to the participants, older adults receive the following:
- Comprehensive assessment
- Weekly support from social work interns who have passed a back ground check
- Opportunities for social engagement and outings
- Address intergenerational and culturally diverse family needs
- Education about disease prevention and health promotion
- Link and or refer to community resources
- Assist in navigating Medicare, Medicaid, and managed care systems
REFERRAL FOR CLIENTS
The eligibility requirements to participate in the Goodwill Neighbors Helping Neighbors (NHN) program include:
- 65+ years old
- Live independently in a home or an apartment
- Live in Salt Lake County
