Access Crisis Team (ACT), Social Worker, GS-0185-9/11, multiple openings. The ACT social worker is a member of a 24/7 interdisciplinary treatment team which provides crisis/mental health services to the Emergency Department, and consultation to outpatient mental health services for new and returning Veterans seeking mental health services. Additionally, they respond by phone to crises in the community and arrange transport and notify authorities when appropriate.

Housing and Urban Development-VA Supportive Housing (HUD-VASH) Social Worker, GS-0185-9/11, multiple openings.  HUD-VASH is the largest permanent supportive housing program in the US.  It utilizes the principles of Housing First to house homeless Veterans and provide wrap around clinical case management and supportive services to help Veterans achieve self-sufficiency.  The program uses a team-based model of care, comprised of multi-disciplinary staff and shared caseloads.  Chronically homeless and other vulnerable homeless Veterans are admitted to case management to support the ongoing effort to end Veteran homelessness.  Where possible, a coordinated entry process is performed in partnership with Community and Balance of State Continuums of Care.

Intensive Community Mental Health Recovery Social Worker, GS-0185-9/11, one opening. The Intensive Community Mental Health Recovery/Mental Health Intensive Case Management (ICMHR/MHICM) Team associated with the VA Salt Lake City Health Care System is an interdisciplinary treatment team which provides intensive case management services to Veterans within the community diagnosed with severe and persistent mental illness. A substantial portion of the workload directed by this position may be carried out at one or more locations that are physically removed from the main unit.

Patient Aligned Care Team (PACT, primary care) Social Worker, GS-0185-9/11,multiple openings. PACT focuses on health promotion, prevention and management of chronic disease. Preventive care and the successful management of many conditions is dependent on the behavioral changes that patients are willing and able to make as well as environmental factors. The role of a social work case manager in PACT is to assess and treat psychosocial and environmental factors that impact the Veteran’s ability to achieve maximum health and wellness.  Social work case managers assess the Veteran’s psychological and emotional adjustment to illness within the context of medical diagnosis, prognosis, and treatment options. An assessment of environmental factors includes a review of the dynamics of the Veteran’s support system, functional status, vocational, economic, housing, spiritual, cultural and legal factors that influence their ability to adhere to medical recommendations and management of self. The social worker assesses the underlying factors that contribute to the presenting concerns and develops interventions designed to promote lasting positive change to decrease stress, promote health and wellness and remove barriers to care.  Veterans are given supportive assistance and referrals to appropriate resources to lessen the acuity of psychosocial stressors.

Substance Abuse Residential Rehabilitation Treatment Program (SARRTP) Social Worker, GS-0185-9/11, one opening. The SARRTP is an integral part of the VA Salt Lake City Health Care System and is aligned within the Mental Health Service Line. SARRTP provides Veteran-centered, recovery oriented psychosocial treatment, nursing, and psychiatric care to Veterans with severe Substance Use Disorders (SUDs) and cooccurring mental illness. The SARRTP has a special emphasis on treating Veterans with substance use disorders and co-occurring mental health disorders.
