Case Management Coordinator

Job Summary

The Asian Association of Utah is seeking a Case Management Coordinator (CMC) to oversee the integration and coordination of psychosocial, educational, and housing services for individuals in the program. Under the direction of the Behavioral Health Clinical Director and Director of Operations, the CMC will lead a team of case managers to enhance and extend culturally and linguistically appropriate case management services to refugees and other qualified migrants as described in the associated grants and projects.

Major Responsibilities

Service Coordination and Leadership:

  • Oversee the integration and coordination of comprehensive services, including psychosocial support, educational resources, and housing assistance to ensure a holistic approach to client care.
  • Develop and maintain relationships with partner agencies and service providers, enduring clients have access to a full spectrum of services.
  • Act as the primary liaison between service providers, clients, and the organization to address barriers to service delivery and ensure smooth communication.
  • Ensure all services are culturally responsive and meet the diverse needs of the client population, in collaboration with leadership.

Service Provision

  • Provide direct case management services to a caseload of clients, including psychosocial assessments, goal setting, and service planning.
  • Engage with clients to assess their needs and provide tailored support plans, connecting them to the necessary community resources.
  • Offer crisis intervention and stabilization services when needed, providing short-term solutions and longer-term referrals to specialized services.
  • Ensure that all services provided are trauma-informed and client-centered, respecting client autonomy and cultural backgrounds.
  • Develop and facilitate psychoeducation groups for clients, covering topics such as mental health awareness, coping strategies, housing stability, financial literacy, and acculturation support.
  • Collaborate with subject matter experts and community partners to co-lead workshops that address specific client needs and build self-sufficiency.
  • Monitor client participation and progress in psychoeducation groups, adjusting group content and format based on client feedback and outcomes.
  • Ensure that psychoeducation services align with the broader goals of the program and contribute to overall client well-being.

Referral Management

  • Assess client needs and make appropriate referrals to community resources such as mental health counseling, substance use treatment, employment services, and legal assistance.
  • Track all referrals to ensure timely and appropriate service delivery, following up with both clients and providers to confirm services are accessed.
  • Create and implement a streamlined referral process, minimizing gaps between service recommendations and service provision.
  • Maintain detailed records of all referrals and outcomes, updating case files and databases to reflect clients' progress.

Data Collection and Reporting

  • Collect client service and outcome data, including service engagement, psychosocial assessments, and client feedback to measure program effectiveness.
  • Monitor key performance indicators (KPIs) related to service delivery and client outcomes, ensuring alignment with grant objectives and program goals.
  • Prepare detailed reports for internal use and external stakeholders, including funders, outlining service outcomes and programmatic impact.
  • Collaborate with the program evaluation team to assess the effectiveness of services and make recommendations for program improvement.

Program Development and Evaluation

  • Continuously evaluate and improve the case management program based on client outcomes, staff feedback, and emerging needs within the community.
  • Assist in developing new service initiatives and program models that respond to evolving community needs, such as additional psychoeducation topics or innovative service delivery methods.
  • Identify gaps in services and work with leadership to secure additional resources or partnerships to address unmet client needs.
  • Stay informed on best practices in case management and community services, applying evidence-based approaches to service delivery and program oversight.

Job Qualifications

Minimum Qualifications

  • Master’s degree in social work (MSW) required
  • Excellent oral and written communication skills, attention to detail, and ability to handle multiple priorities and interpersonal skills.
  • Demonstrated appropriate decisiveness and creative problem-solving when making decisions regarding next steps for cases.
  • Access to a personal vehicle, proof of valid driver's license, auto insurance, current registration, good driving record and passing a background check is required.

Preferred Qualifications

  • Experience working with refugees and immigrants and/or lived experiences within newcomer communities and understanding of their unique needs
  • Preference given to bilingual individuals

Compensation Package

  • Salaried non-exempt position
  • Full medical, dental, and vision benefits
  • Paid Time Off earned at 6.5 hours per pay period
  • Retirement package with agency contribution of 6%, no match required; available after 1 year of employment

Job Type: Full-time

Pay: $23.00 - $30.00 per hour
