Instructions for Requesting Social Work Undergraduate Permission Codes
(1000-5999 level SW courses for BSW and SUDC programs)
Please note that the College of Social Work can only issue permission codes for SW
classes. If you require a permission code for a class offered outside of the College
of Social Work, please contact the department offering the course for assistance.
Please do not email Social Work faculty or staff for permission codes. Students must
use the form below.

Please do not send multiple requests for the same permission codes. This delays our
ability to help all students in an efficient manner.
Requests are processed in the order received. Please allow up to one week (5 business
days) for your request to be processed.
When you receive a permission code, please remember that is good for single use –
it is no longer valid after a student uses it once. Thus please exercise care when
adding your classes, and please do not share permission codes.
Students are not guaranteed to receive a permission code – it is crucial that you
