Social Worker

This Social Worker position performs work in providing guidance to older adults and their families regarding available resources and alternatives that help maintain seniors in the home. Manages in-home service programs and coordinates family and community support efforts for older adults seeking resources to enable them to remain independent. This is an Alternative Funding Position.

Rate of Pay:  $28.16/hourly

Example of Duties:

Schedules appointments with older adult clients and their family members to determine eligibility for services offered within various programs.
Completes regular follow-up client evaluations; revises care plans as needed and acts as a liaison and advocate for clients.
Completes a functional and social assessment of each client.
Maintains records on types of services being utilized; documents client contacts; tracks service units on all clients/programs; compiles reports; approves monthly billing.
May become an Ombudsman for long-term care facilities.
May assist in Senior Health Insurance Programs such as Medicare and fraud awareness.

Minimum Qualifications:

A. Bachelor's degree in social work, sociology, psychology, family sciences, gerontology, or a closely related field.

AND

B. Current Social Service Worker or Licensed Clinical Worker License with the Division of Occupational and Professional Licensing

PLUS

C. Two (2) years of related work experience

 

Required Knowledge, Skills, and Abilities:

Working knowledge of current social work practices and working knowledge of psychology and sociology. Ability to establish and maintain effective working relationships with other employees, other agencies, and the public; ability to follow written and verbal instructions, ability to communicate effectively, both verbally and in writing. Must have a valid Utah Driver's License. The employee must occasionally lift and/or 20-25 pounds with occasional lifting up to 100 pounds or more.
