Health Care Navigator

What would you do?
As a Health Care Navigator you would be helping veterans and their families who are either literally homeless or at risk of homelessness in order to coordinate them with medical support. This includes helping apply for VA medical, or Medicaid/Medicare, obtaining Primary Care providers and Mental Health providers for the veteran or family members, and coordinating to ensure the participants have full access to available medical assistance. You will be following up with the veteran to assure they make their appointments, following the medical provider’s recommendations, etc.

Compensation and Benefits:
$21.00 - $27.00 / hour – based on experience
Paid training to become a Certified Case Manager and SOAR Certified

Who you are:
Passionate about helping people
Able to understand health care needs and make a plan to address needs
Experience coordinating individuals with hospitals, clinics, private practice etc. preferred
Organized and detail oriented with strong computer skills
Able to adapt to new situations
Able to do a night or two of travel occasionally based on case load
Able to take a rotating on-call phone that is split between case managers in 1 week increments
Must have a clean Motor Vehicle record and background check
Willing to transport Veterans to appointments when needed
Bachelor degree required, Master’s preferred in Social Work, Social Sciences, Public Health Administration, Psychology, or related field.
May have prior experience in Social Services
Veteran Preferred, MSW or MPH preferred.
This is a non-licensed position as the individual will not be doing diagnosis, medical assessments, or administering any type of medical services.
