PS Social Worker/Case Manager

Job Summary
The Intensive Outpatient Clinic (IOC) at the University of Utah is expanding, and we are looking for a licensed clinical social worker to provide professional counseling services and case management services to a population of individuals with complex psychosocial needs. This is an exciting opportunity to provide direct therapy as well as assist with developing a comprehensive behavioral health program aimed at treating the bio‐psycho‐social needs of IOC patients. Therapy activities include individual, group and couples counseling. At a minimum, the ideal candidate will have working knowledge of trauma and substance use disorders. Case management services include monitoring patient care to ensure progress toward desired outcome, addressing patient and family needs, resolving obstacles to effective care, coordinating care with payers and vendors, patients and families. This position is considered Category I: Mandatory and required on campus or health care facilities to support on‐site
operations. Employees hired into this category will be required to be physically located at 2830 S Redwood Road, West
Valley City, UT 84119 if they are healthy and without COVID‐19 symptoms.
We work under a capitated model, so no billing required. Professional counseling services will be provided to a specific panel of patients and case management services will be provided for the whole IOC patient population.
1. Develops and implements individualized treatment approach to meet the needs of each patient and their families.
2. Works in close collaboration with all members of the treatment team including medical doctors, to further their
1. understanding of significant social and emotional factors contributing to the patient’s well‐being.
2. Identifies appropriate outside resources, such as community agencies, to assist patients with resuming life in their
3. own community and how to live with the patient’s limitations.
4. Assists in the development of departmental policies and procedures.
5. Oversees and supervises Social Workers, Interns and caseworkers within area of assignment.
  1. Routinely assesses client and family response to services. Identifies patient needs, including those of an ethical and cultural nature, and ensures they are addressed.
  2. Assesses and discusses funding and insurance issues with client, family, and healthcare providers to enhance cost effective utilization of services and quality outcomes.
  3. Negotiates with third party payers relative to benefit levels, eligibility, utilization review, and reimbursement.
  4. May be required to complete home or site visits as required by the department.
  5. Help patients overcome needed social determinates such as but not limited to: housing, transportation, communication, insurance changes, finances, and food options.
  6. Demonstrated independent judgment to assess and meet client needs.
Minimum Qualifications
Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.
One of the following
Current Licensed Clinical Social Worker (L.C.S.W.) certificate for clinical practice in the State of Utah.
Current license to practice as a Certified Social Worker in the State of Utah.
This position is patient-sensitive and must fulfill all associated requirements. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.
