Position Summary: The Disabled Rights Action Committee is a small case management agency in Salt Lake City Utah. We help individuals with disabilities move from institutional settings into integrated community-based environments and help to ensure that their needs are safely met. Our organization also works to establish equal rights for people with disabilities through enforcement of federal and state laws.

In this role, you will be working with our existing case management staff to respond to client needs as they arise. This typically amounts 5-20 hours per week (depending on your availability), though more clients/hours may become available as our program grows. Training will take place in person through Zoom.
Primary Responsibilities: Complete initial comprehensive assessments and periodic reassessments to determine the services and supports required by clients to prevent unnecessary institutionalization.

Research the availability of non-Medicaid resources needed by clients.

Act as the client’s advocate as it relates to insurance coverage and financial assistance.

Assist clients in gaining access to available Medicaid State Plan services.

Locate appropriate home and community-based settings and assist with negotiation of rental agreements when needed.

Maintain the client’s record through detailed documentation.

Monitor to assure provision and quality of services.
Desired Qualifications: Bachelor’s degree in Social Work (minimum)

Licensed as a Social Service Worker (SSW), Certified Social Worker (CSW) or Licensed Clinical Social Worker (LCSW)
1-3 years of experience, preferably in case management or related area (an active license is required)
