Permanent Supportive Housing Care Manager

The Road Home has been a leader in the fight to end homelessness for almost a hundred years. We provide low barrier emergency shelters, supportive services, and housing-first minded rental assistance that helps individuals and families step out of homelessness and back into the community. We are seeking compassionate and empathetic individuals who are interested in making a difference in the lives of others and their community.

The Permanent Supportive Housing Case Manger will provide trauma-informed, guest-driven case management services to individuals who are moving out of homelessness with the goal of helping them achieve long term stable housing. The services provided include connecting them with resources in the community that will aid in their transition into housing. The Permanent Supportive Case Manager will work with other agency teams to identify common barriers faced by individuals we serve and help with establishing strategies and procedures for addressing needs on an agency-wide scale.

Housing First
• Provide outreach services to program participants in their homes, emergency shelter, and other locations.
• Assist guests in identifying barriers and connect them with appropriate resources in the community.
• Strong belief of the Housing First model.
• The ability to engage and encourage housing conversations with guests.

Managing a Case Load
• Assisting our diverse population of guests on your set caseload to establish goals. • Assist tenant in obtaining employment and or working on applying for Social Security benefits. • Focus on housing and strengths-based practices to meet the unique needs of each tenant. • Assess children’s needs to make appropriate community referrals and liaise with schools as needed for supportive services
• Daily collaboration with all departments promoting a healthy team environment.
• Seeking out, engaging, and networking with community partners to find housing options and additional resources.
• Attended weekly team meetings and contribute to case conferencing conversations.
• Keep accurate records, case management notes, and statistical data as required and participate in special projects as needed.

Resource Collaboration
• Provide direct, trauma-informed case management services to guests, including outreach, ongoing assessments, goal setting, creative problem solving, using a network of community resources.
• Collaborate with the crisis clinician and others to find ways to support individuals with mental and behavioral health barriers.
• Ability to accept direction and constructive feedback from Supervisor.

Promoting Best Practice
• Demonstrates awareness and desire to understand diverse populations.
• Ability to contribute to the Agency's commitment to enhancing awareness and appreciation of diverse ethnic and cultural heritages.
• Willingness to understand that housing first, harm reduction, low barrier access to resources is best practice and essential to our mission at The Road Home.
• Ability to set and maintain good boundaries, using Trauma-Informed Care as a guideline

Experience
• Lived, work or volunteer experience preferred or a degree in a related field
• Experience working or volunteering with diverse populations
