Medical Outreach Case Manager

Job Summary
University of Utah’s Moran Eye Center is seeking a full-time Medical Outreach Case Manager. You’ll gain valuable experience in a medical setting and have a chance to work side by side with world class professionals. This position will work under the supervision of a PhD, LCSW director of the Patient Support Program.
    • Conducts in-clinic or telephone assessment of psychosocial needs of visually impaired patients in an out-patient ophthalmology setting.
    • Develops individual plan of support through collaboration with the patient’s physician.
    • Provides information and referrals for community resources to patients and interacts effectively with community agencies and service providers.
    • Provides advocacy to patients to meet changing service needs.
    • Provides patient education of community resources and individualized education of life skills.
    • Documents patient contacts and services provided. Interacts and collaborates with medical faculty and staff on behalf of patients.

Work Environment and Level of Frequency that may be required: 
Nearly Continuously: Office environment.
Seldom: Infectious disease, oils (there is air or skin exposure to oils or other cutting fluids).
Physical Requirements and Level of Frequency that may be required
Nearly Continuously: Hearing, listening, talking.
Often: Repetitive hand motion (such as typing), walking, standing, sitting.
Seldom: Bending, reaching overhead.
Minimum Qualifications
Bachelor’s degree in human or social services or equivalency required. Demonstrated human relations and effective communication skills also required. 
Fluency in the Spanish language is preferred. 
This position is patient-sensitive and must fulfill all associated requirements. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.
