Case Manager

Job Summary 

This position will consist of various case management and social work/mental health related tasks. Specifically, this position will oversee the referral process of individualized mental health service provisions, serve as a liaison between the Utah School Mental Health Collaborative leadership team, Utah schools, and community service providers. You will also be responsible for facilitating open and accurate communication and coordination among school-aged students, their families/caregivers, referral providers, and the school Well Being teams (composed of University of Utah Graduate students). This is a collaborative position between the University of Utah and various Utah School Districts, it may require travel to schools (physical or virtual) and working with a team of graduate student supports and university staff.

Responsibilities 

Essential Functions
 ●      Attend and contribute to the bi-monthly meetings with each school team to discuss management, implementation, and coordination of mental health and wellbeing services.
●      Initiate and coordinate community mental health partnerships with school districts. 
●      Liaise with caregivers, community providers, teachers, and WBT on individual students’ cases.
●      Evaluate system-wide issues with access to MH care within the school community, collaborate with school representatives (e.g., school social workers & administrators) and provide recommendations for amelioration of these issues.
●      Coordinating with internal psychiatric & medical providers to ensure appropriate supplemental support to help with their service provision. 
●      Advocate for students from low-SES & other underrepresented families through identification of and connection with free and reduced-cost services that support MH and wellbeing directly to the student (medical, psychological assessment not otherwise provided by the team, more intensive in- and out-patient treatment options, etc.), and indirectly through resources provided to caregivers (connection with Division of Services for People with Disabilities, Department of Workforce Services, etc.).  
●      Coordinate holistic student care with outside providers – direct communication, securing/providing records through proper ROI channels.
●      Coordinate with outside agencies as needed (e.g., DCFS) – including caseworkers, law enforcement agents, and others as appropriate.
●      Scheduling and coordination of psychiatric evaluations and coordination/follow up with Primary Care Physicians. 
●      Communicating with supervisors and other co-workers in a timely and professional manner.

●      Functions as the patient advocate to facilitate effective communication and appropriate treatment.
●      Orients the patient to the Health Sciences Center program.
●      Works with the health care team and patient in the development and implementation of a care plan.
●      Advises patients and families regarding financial matters and assists social workers and case managers in accessing financial and community resources.
●      Assists in establishing and maintaining good relationships with referring physicians, third party payers and community agencies.
●      Facilitates the involvement of team members, timely completion of evaluations and reports, and the scheduling of team conferences.
●      Maintains contact with patient families in order to facilitate active involvement and knowledge of patient progress.
●      Contacts payers and submits reports regarding the patient’s initial assessment, expected outcome, expected cost and then recommends a discharge plan.
●      Ensures the payer is represented in staffing conferences when requested.

Minimum Qualifications 
 
Required
●      CSW preferred. 
●      MSW or Bachelor’s degree in social work with clinical practicum experience
●      Two years of experience in case management activities with systems-level work 
 
Additional Qualifications
Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah. Must maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah.
 Current Licensed Clinical Social Worker (L.C.S.W.) certificate for clinical practice in the State of Utah.
 Current license to practice as a Certified Social Worker in the State of Utah.
 Current license to practice as a Clinical Mental Health Counselor in the State of Utah.
 Two years of professional experience in a clinically related area.
 Dependent upon the department of hire, may be required to provide reliable transportation for site visits.

Preferences
 
Preferred
●      Current licensure to practice in a clinical field may be preferred.
●      Collaborates with social work/nurse case manager and others to confirm authorization for services is in place.
●      Assists in collecting data related to patient outcomes and auditing data for accuracy.
●      Prepares documents and reports required for internal and/or external groups in the scope of care coordination activities and/or data collection.
●      Documents activities and progress in medical charts, computer billing/utilization systems, or in other ways as directed.
●      Collaborates with internal and/or external partners to address financial advocacy for patient/organization.
●      Inputs clinical information into computer database.
●      May be required to participate in quality improvement initiatives per the department.

Additional Preferences  
Basic Life Support Health Care Provider card through American Heart Association. 
A Certified Case Management designation. 
Previous experience in clinical resource management activities and third party payer interactions.
