Social Services Supervisor/Resident Advocate

FLSA Status: Exempt/Non-Exempt

Salary Range: $20-28

Expected Hours: 9-5, M-F

Summary of Job: The Social Services Supervisor is responsible for assisting residents to manage their everyday physical, mental, and psychosocial needs. They are an advocate for resident needs and rights regardless of the resident’s background or previous life choices and as such is expected to treat everyone with respect. The supervisor will also manage other members of the Social Service/Resident Advocate staff.

Examples of Duties and Responsibilities

  • Makes arrangements for obtaining needed adaptive equipment, clothing, personal items, counseling services, etc.
  • Maintains contact with family, in consultation with the resident, to report on changes in health, goals, discharge planning and encourages their participation in care planning
  • Makes referrals and obtain services from outside entities, i.e., talking books, absentee ballots, facility wheelchair transportation
  • Assists residents with financial and legal matters, i.e., Social Security, VA, referrals to funeral homes for preplanning arrangements
  • Builds relationships between residents and staff and teaches staff how to understand and support resident’s individual needs. Also promotes actions by the staff that maintain or enhance resident dignity in full recognition of each resident’s individuality
  • Ensures confidentiality of resident’s social services information by allowing access only to authorized persons
  • Participates in Interdisciplinary Care Plan Conferences and develop plan of care to ensure that special social service needs are met
  • Assists in resolution of grievances as voiced by resident, family, responsible party or ombudsman
  • Completes Social Service Assessment Discharge Planning and MDS section in EMR system for each resident within the required timeframe according to facility policies and procedure, or more frequently if needed
  • Documents in the resident’s plan of care all identified social service problems with appropriate approaches and time measurable goals
  • Other duties as necessary and assigned


  • One year of supervised social work experience in a health care setting, preferred
  • Experience completing electronic medical records
  • Basic computer skills including Microsoft Word and Excel

Experience and Education

  • High School Diploma or equivalent
  • Bachelor’s in social work or human services
  • Preferred Masters Level license in Social Work
  • CSW or LCSW helpful but not required

Physical Requirements and Environmental Conditions

  • Exposure to body fluids and communicable diseases.
  • Full range of body motion including handling and lifting objects.
  • Ability to lift/push/pull 50-100 lbs. unassisted.
  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met
